Fee Policy
Patients are responsible for any fees not covered by insurance or for lapses in insurance coverage during treatment.

Co-payments must be paid at the beginning of treatment. There will be a charge of $5.00 assessed for late and billed co-payments. In the event of a insufficient bank funds charge, there will be a $15. 00 fee assessed.

Cancellation Policy
It is required that you provide us with a minimum of 24 hours cancellation notice to avoid the Full session fee (unless there is an emergency or illness)

Insurance Policy
Ann M. Porto, Psy.D. & Associates, Inc. accepts many insurances including United Health, Medicare, Blue Cross and Blue Shield products. Please call your insurance customer service number to make sure that your insurance covers the service you are seeking in our practice.

Office Hours
The office is open six days weekly. There is evening hours and Saturday day time availability in the event of need for some but not all therapists.

You must be scheduled during normal business hours (Monday-Friday 9:00 Am -3:30 PM) in order to be screened for all required information by the secretary at the practice, to determine your appointment time and therapist assignment.
